Position Overview

The Doctor – Sub-Acute Care & Rehabilitation will manage medical care and rehabilitation for long-term geriatric patients and those recovering from post-surgery, acute hospitalisation, or chronic conditions such as dementia or stroke.

You will work closely with the MDT, ensuring seamless rehabilitation pathways, safe discharge planning, and high-quality clinical outcomes. This role offers the chance to provide patient-centred, collaborative care in a stable, well-resourced environment.


Key Duties & Responsibilities

  • Conduct comprehensive medical assessments on admission (physical exams, history review, diagnostics)
  • Develop and implement individualised treatment plans, including medication and rehabilitation strategies
  • Collaborate with the MDT (physiotherapy, occupational therapy, speech therapy, dietetics, social work, and nursing)
  • Monitor patient progress and adjust care plans as required
  • Communicate with patients and families on conditions, treatment plans, and self-care strategies
  • Maintain accurate medical records on Healthware (electronic medical platform)
  • Participate in quality improvement initiatives and protocol development
  • Provide urgent/emergency telephonic medical support to nursing staff when necessary
  • Establish and maintain professional relationships with referring specialists in the Tokai catchment area

Requirements

Qualifications

  • MBChB (Bachelor of Medicine & Bachelor of Surgery)
  • Active HPCSA registration
  • Valid MPS / medical indemnity insurance

Experience & Knowledge

  • Post-community service experience required
  • Experience in internal medicine, family medicine, geriatrics, or palliative care is advantageous
  • Proven ability to work within a multidisciplinary healthcare team
